The B.P. Eye Foundation Hospital for Children’s Eye, ENT, and Rehabilitation Services, is at a pivotal moment in the advancement of eye care services. While it has long relied on Small Incision Cataract Surgery (SICS) as the primary method for managing cataracts, we now seek to transition to a more modern, efficient, and patient-centered surgical approach: phacoemulsification (Phaco). This transition represents not just an upgrade in technology but a leap forward in quality, accessibility, and impact. Phacoemulsification uses ultrasonic energy and a micro-incision (typically 2-3 mm) to remove cataracts, enabling faster healing, reduced surgical trauma, and improved visual outcomes. It allows for the use of foldable and premium intraocular lenses (IOLs), making it especially beneficial for children over five with disabilities, whose cognitive and physical development is closely tied to restored vision. In contrast to the larger incisions of SICS (6-7 mm), Phaco procedures are less invasive and enable quicker recovery, shorter hospital stays, and better integration into rehabilitation and educational programs. This shift will directly benefit over 4,800 individuals annually, including children, adults, and the elderly, many of whom are from underserved areas such as Madhyapur Thimi-1, home to approximately 120,000 people. By reducing wait times and increasing surgical throughput, Phaco will significantly enhance our ability to deliver timely, high-quality care. More than a technological shift, this is a strategic and sustainable investment in public health. Cataract remains the leading cause of avoidable blindness in Nepal, accounting for 82% of cases. By introducing Phaco, we aim to reduce preventable visual impairment, promote social inclusion, and support broader development goals such as good health, quality education, and reduced inequalities. Objective Strengthen eye care services by introducing phaco-based cataract surgery, improving access, enhancing surgical quality, and supporting particularly the people with cataract . Key Beneficiaries: 1. Preschool and school children with disabilities. 2. Adults and the elderly. 3. Families, or caregivers 4. Local communities in Bhaktapur and the surrounding areas
